What to Bring to Your Appointment

To ensure your visit goes smoothly, please bring the following items to your appointment.

  • Photo ID (driver's license or state ID)
  • Insurance card(s)
  • Referral paperwork (if required by your insurance)
  • List of current medications
  • Any relevant imaging (X-rays, MRI, CT scans)
  • Previous medical records related to your condition
  • Completed patient forms (if provided in advance)

Arrive Early

Please arrive 15-20 minutes before your scheduled appointment time. This allows time for check-in and any necessary paperwork.

New patients: Please arrive 30 minutes early to complete registration forms.

Please call our office at least 24 hours in advance if you need to cancel or reschedule your appointment. This allows us to offer the time slot to other patients who may need care.

How long will my appointment take?

Initial consultations typically last 30-45 minutes, while follow-up appointments are usually 15-20 minutes. Your provider will take the time needed to address your concerns thoroughly.

What if I need imaging (X-rays, MRI)?

X-rays can be performed in our office during your visit if needed. For MRI or other advanced imaging, we will provide you with a referral to an imaging center.

Do you offer payment plans?

Yes, we offer payment plan options for patients who need them. Please speak with our billing department to discuss available options.
